General Guidelines of Revive Fitness

Registration, Waiver & Consent Forms must be completed before participating in any program at Revive Fitness.
Initial FREE class may be used one time only for Group Classes only.
Consultations, Body Fat Measurement Analysis, Specialty Workshops or Clinics, Life and Nutritional Coaching available for additional fees.
Schedules, instructors & hours subject to change with notice. Holiday closures, reduced schedules & shut down days have been included into pricing. Revive Fitness is closed all Statuatory Holiday weekends and/or days, Christmas & March Breaks (unless otherwise stated). Closures due to unforseen cicumstances or inclement weather not refunded.
Due to safety, children may not attend programs while parents are training. Childminding services are not available at the facility.
Revive Fitness is not responsible for any articles left on the premises.
Towels and water are suggested as we do not provide them.
No purchases can be transferred, put on hold or refunded for any reason. Canadian Protection Act 2002 does allow for a 10 day cooling off period.
We do not reimburse any membership, program or service time that has not been paid for by the member as part of a promotion or donation to a charity.
Referral Program, refer a friend to sign up for any program or membership and receive a $20 Referral Bonus as our Thank you to you.
When you become a member at Revive Fitness, we collect information that is essential for the operation of health provision. Information received via our web server automatically collects your internet domain name or IP address which may identify you depending on the “naming standards” followed by your Internet Service provider. This information is collected so that we know how many people visit our site, the geographical region they are from, and which pages they view on our site. It is maintained in our confidential statistical reports, so individuals are not identified, and is never given out or sold. If you use a feedback form or contact us, you will be providing us with some personal information including your name, address, email address and perhaps information about your personal circumstances. This information will be used only to reply to your question. None of the collected information will be disclosed to any other office or individual outside Access and Privacy Services.
Cancellation / Refund Policy
Revive Fitness has the right to terminate membership for default in payment, delete all members contents without notice, for conduct Revive Fitness believes harmful to Revive Fitness business and if Revive Fitness feels the need to terminate a contract there will be no refund of fees.
3 or 6 month Term Memberships are to be paid in full upon registration, financing is not available for this option.
12 month Memberships have financing available with post dated monthly cheques or credit card. Cheques to be made payable to Revive Fitness and posted for the 1st of each month. Credit Card payments are posted on the 23rd of each month. In the event of a default payment, at its discretion, Revive Fitness may declare whole payment is necessary to continue the contract.
Only 12 month Memberships MAY be frozen, please see “Freeze” policies below.
12 Month Memberships may not be put on hold for any other reason than major medical issues.
For major medical reasons or medical emergencies, we offer:
Freeze available for 12 month Memberships in good standing only.
Minimum of 30 days and maximum of 3 months. For insurance purposes, a doctor’s note to start the freeze and end the freeze is necessary to utilise the freeze offer. An additional $25 administrative fee is collected up front for each start and stop of the freeze. Failure to provide the doctor’s notes will not extend the 3 month maximum.
Payments do continue for the duration of the contract term and the frozen months will be added to the end of the contract extending it to the number of days/weeks/months of the approved freeze. Please contact the Revive Fitness Manager, Lee-Anne Simpson for Approval.
Team Training-Personal Training-Specialty Policies
Sessions cannot be put on hold or transferred for any reason.
24 hour Cancelation Policy in effect. If a cancellation is not provided within this time period a charge for the session will be posted to the account.
Rescheduling of Team or Personal Training (private or semi private) will only be honoured if Revive Fitness can accommodate the request or the session will not be made up.
Team or Semi Private Personal Training – if an individual participant is unable to attend their scheduled session there is no make up or refund for the missed session.
Any participant under the age of 18 years requires consent from their legal guardian.
