Payment Policies: (All Payments in Canadian Dollars)
- Cash & cheques credit cards accepted.
- Post dated cheques for monthly Adult/Youth Membership dues to include Administration Fee of $99 plus pro-rated amount for the first month.
- 13% HST has been included in pricing.
- $20.00 NSF fee for reversed payments.
- All schedules & instructors subject to change with notice. Holiday closures & shut down days have been included in scheduling & prices. Revive is closed all
Statutory holiday weekends and/or days, Christmas & March School Breaks (Unless otherwise stated). Closures due to unforeseen circumstances or inclement weather not refunded.
- Memberships cannot be transferred, put on hold or cancelled.
- Memberships can only be “Frozen” with medical documentation and with 1 month written notice. $20 Admin Fee applicable.
- Upon default in any of the scheduled payments the Studio may at its discretion declare the whole payment to be made in full.
- Membership sure for membership renewals are due 1 month prior to expiration date of the annual contract in order to avoid the $99 Administration Fee.
Facilities Policies:
- Proper gym attire and INDOOR running shoes must be worn
- Memberships, Personal Training, Team Training and Specialty Program registrations cannot be put on hold or transferred.
- Please allow 48 hours notice when cancelling any Private Training Sessions.
- There is a no-cancellation/makeup Policy in effect for private and team training sessions. NO CANCELLATIONS OR MAKEUPS ALLOWED.
- Annual Memberships require $99 Admin fee to be added to the first month current dated cheque along with 11 postdated cheques for the 1 st of each month.
(We understand sometimes you need to order cheques - so will allow 14 days for all cheques to be received - any cheques received after the 14 days - or if you require a reminder - will cost $20 per incident.)
- Increasing or decreasing a membership package must be done with 1 month written notice and an administration charge of $20.00 will be applied.
- Prices subject to change without notice. Contract fees are maintained throughout the contract.
- Any participant under the Age of 18 requires consent from their LEGAL GUARDIAN.
- Waivers and Consent Forms must be completed prior to the start of any training.
- Due to Safety, children may not attend programs while parents are training.
- All classes will begin at the time specified on the schedule.
- All classes are co-ed, unless otherwise specified.
- Schedule & hours subject to change at any time, with notice.
- ReVive Fitness is not responsible for any articles left on the premises.
- Towels and Water are suggested.

PRIVACY POLICY
When you become a member at ReVive Fitness, we only collect information that is essential for the operation of health provision. Information received via our web server automatically collects your internet domain name or IP address, which may identify you depending on the "naming standards" followed by your Internet Service provider. This information is collected so that we know how many people visit our site, the geographical region they are from, and which pages they view on our site. It is maintained in our confidential statistical reports, so individuals are not identified, and it is never given out or sold.
If you use our feedback form or contact us, you will be providing us with some personal information including your name, email address, and perhaps information about your personal circumstances. This personal information will be used only to reply to your question. It will not be disclosed to any other office or individual outside Access and Privacy Services.

Freeze Policy
We understand that under certain circumstances you may need to freeze your membership.
We allow a membership suspension or freeze for the following reasons with $20 Freeze Fee.
- You are under a physicians order not to exercise. A doctor’s note is required to begin a freeze and unfreeze your membership.
- 3 months for emergency medical – doctor’s note is required to freeze and unfreeze.
Please note
- Freeze requests must be longer than 30 days.
- Freeze requests will only be authorized for regular memberships of a term longer than 1 year
- Memberships must be in good standing (if paid monthly through automatic dues) or paid in full at the time of request.
- Under no circumstances will we back date a freeze. The freeze date will begin the day the freeze form is signed, documentation is given and freeze fee is paid (if applicable).
- Freeze requests will not extend past your membership contracted term.
- All membership time will be given at the end of your membership agreement. Please be aware that your monthly dues will continue through your freeze.
We will not reimburse any membership time that was not paid for by the member as part of a promotion or donation to a charity. |
